Error code 12 means INSERT_ERROR_UNKNOWN - so we cannot determine the reason without any further details. What exatly are you trying to do? Maybe the diagnostics output will give us a hint an what's going on: right-click on the Quick Start Icon -> Virtual CD Programs -> Show diagnostics.
